signup failed borked error
 

hi there i am wondering if anyone would have a solution for this silly little error i have searched not only this site many others and reinstalled windows and everything is clean install it all went in successfully and site opens but when i reg as sysop as first account i keep getting this error signup failed borked
this is the source i have in i had it in before so i know it works just not sure what the problem is with the takesignup.php to generate this error
