That will probably stop the notice.

You need to make sure the form name is the same as the $_POST

Eg.

<td><strong>Genre:</strong></td>
<td><input type="text" name="gen" class="tv">&nbsp;&nbsp;<strong></strong></td>

The name of the form is "gen" so the $_POST will be $_POST['gen'] , so check you page and make sure they match.
